Forms

This page is the initial page that will host or link to many of the most important forms for graduate students here at UNC-Charlotte. If any of these links should stop working, please try the SAFC website mentioned below and send a message to gpgstreasurer@uncc.edu to let us know. 

Administrative Forms

For information on how to appropriately fill out the Chartering and Re-Chartering Forms, contact the GPSG Vice-President at gpsgvicepresident@uncc.edu.

  • Chartering Form - Use this form to charter a new graduate organization with GPSG. (Format: MS-Word/ Size: 35 KB)
  • Re-chartering Form - Use this form to re-charter an existing graduate organization with GPSG. (Format: MS-Word/ Size: 27 KB)
  • Registration & Re-registration Online Form - Use this link to student organizations to register a graduate organization with SGA.
  • Operational Fund Request Form - Use this form to request an operational fund if you re-chartered after the deadline.
  • Contact Information Form - Use this online form to update your group's officers and their emails so that the current officers will get the info that GPSG sends out to the groups and not last year's officers.
  • Volunteer Proposal form - This form must be filled out prior to your group's volunteer event. Please contact the secretary with questions concerning point requirements.

Travel Forms

For information on how to appropriately fill out the travel forms and what deadlines you need to know, see Travel Forms How-To.

Pre-Travel Forms:

Post-Travel Forms:

  • Travel Settlement Form - To be completed for reimbursement of approved travel. Must attach all receipts.
  • Travel Evaluation - To be completed with the travel settlement form after the travel. (attached to Settlement form)

Event Forms

For information on how to appropriately fill out the event forms and what qualifies as an event, see Event Forms How-To.

  • Event Application - This form is to be used to request money for any event being held.
  • Conference Evaluation - (only for conferences) To be completed after the event.  Use it to explain how successful the conference was in relation to the host group's expectations.

Other forms you may need for your event:

  • Contract Information Form - Use this to request that a legal contract be written for a speaker/lecturer who you are hiring. This is necessary if you are inviting someone to speak and are paying them. (Format: Excel/Size: 34KB)
  • Request for Reimbursement Form - This form is to be used to request reimbursement for most other types of expenditures.

Flyer/Advertisement Forms

  • GPSG/SAFC Logo (pdf or jpeg) - This needs to be on all flyers/ads for events which use money from GPSG
  • Advertisement Guidelines - lists what to include in your advertisement strategy for events sponsored by GPSG  (coming soon)
